Could be a throwout bearing... The bearing should not be touching any rotating assemblies when the clutch disc is engaged to the flywheel/pressure plate (pedal is released). If the clutch is maladjusted, the bearing can ride against the pressure plate fingers, causing the bearing to spin at all times, and it will fail prematurely (very prematurely). Also keep in mind that maladjustment can cause the pressure plate fingers to contact the disc at full disengagement. In other words, if the throwout bearing gets pushed too far, it can cause some very nasty, noisy interference.
