What to do about this? Can I just sandblast it or I need to take it completely apart?

If I go down sandblasting route is there a possibility that sand might get inside and cause damage later?

Depends on your desired outcome. If this car is a resto or high end build then blast it as it is with emphasis on the 3rd member. Then disassemble it; have the housing blasted, thoroughly hose it out and replace all the seals. Make sure to rinse out the 3rd member in a parts washer. Put it all back together and paint it.
