Numbers for disc brake swap equipment

About 2 years ago I bought disc calipers off of a 73 Duster for my Challenger. The guy was on an a-body site and at the time gave me all the numbers for the hoses, rotors brakes, attaching hardware and basically everything I needed to put the stuff on my car. Well my computer crashed and with it went the list and all that valuable information. I know I need to get the caliper adapters off a 76 or so Cordoba or other car of similar vintage to put the 12" rotors on. Does anyone have a complete list of the parts needed? I have manual drum right now and want manual disc when finished. I plan on eventually doing rear disc also but thats way down the road. Thanks in advance.   :popcorn:
