make, Model ??

It would be a Carter AVS.  I don't have a number here at this time.  A different carb was used for automatic and a different carb was used for the 4 speed.
1969 Sport Satellite H code convertible, 1970 Cuda 440+6, 1970 Challenger R/T 440+6, 1970 Challenger 383 R/T auto, 1970 Challenger R/T 383 4 speed,1971 Challenger convertible.

some use small 600 cfm Holleys but definatly not the ultimate for performance , more 383 s used the Holley

According to "Mopar Broadcast Sheet Decoding Manual"...

440 4bbl high perf w/o E.C.S. is Carter 4737S for manual trans and 4738S with auto trans

If you have E.C.S., then it is Carter 4739S for manual trans and 4740S with auto trans

If you have a/c and auto trans, but no E.C.S., it is Carter 4741S.

Holleys were used on the std. perf. 440 motors this year according to the book. Holleys were also used on the 383 perf. motors this year.

I used this book to break down everything on my super bee's broadcast sheet and it was right on the money on everything. By the way, this book mentions codes for just about every factory part Mopar bolted onto our cars.
