Fan clutch failure

Local Mopar club member has an issue I thought someone here might help with.  Any idea what caused this?
  I heard this loud bang from the front of my 69 gtx hood blow up like there was pressure under the hood (radiator hose, right).  The radiator fan was bolted to the back of the clutch and what had happen was that 2 of 8 bladed fan blades separated/ broke at the bolts on the back of the clutch.

I had a clutch fan that had some play , if you held onto each side of the fan you could pull one side forward myabe 1/4 " , on a long trip it broke the water pump shaft leaving me stranded with an unfixable rad & nothing to replace the rad with down in Nebraska it is worth replacing old clutches

CP I could see a catatophic clutch failure but the way I read this is the fan blades broke off from their bolt area ....maybe the result of failure somewhere else?!?!

  Was the fan new? or has the fan been bent and straightened?

  I have seen blades break off and go through the hood after somebody had attempted to straighten them, fatigue.
