My gas pedal shaft slipped out of the bracket. I found a washer on the carpet but nothing else. What is supposed to secure the shaft so it does not slide out of the bracket? There did not seem to be anything on the end closest to the brake pedal and I can't find a hole in the shaft for a cotter pin.
