    Looks like the concrete was to thin.  They have specs for the thickness and that looks thin.

    The concrete anchor bolts needed to be much longer or poured into a footing made for that much weight.

    Driving and saw something sticking up from on top of a truck, thinking it may be of importance they should know, but as I got closer apparently they already know. Zoom in on the lid sticking up, it says "We know" Got a little chuckle out of that.
